I developed a web application to record staff adherence to the Code of Conduct of the Comisión de Operación y Fomento de Actividades Académicas del Instituto Politécnico Nacional (COFAA-IPN). The project addressed an audit request to keep records of staff awareness of the code. The application stored acknowledgments after employees consulted the document and enabled administrators to retrieve and download the records.

Problem

An audit requested a record of staff awareness of COFAA’s Code of Conduct. The institution needed a process for consulting the document, recording each person’s adherence and gathering acknowledgments for administrative review.

Solution

I created an application using PHP, MySQL, HTML, CSS and JavaScript. Staff consulted the Code of Conduct and completed a form with their details. Their acknowledgment was saved in the database and a copy was sent by email. Administrators could retrieve and download individual acknowledgments and a consolidated XLS file.

    I integrated the document into the application so staff could review it before completing the registration form.

    I implemented delivery of an acknowledgment copy by email after the user completed the form.

    Administrators could retrieve and download each user’s acknowledgment and an XLS file consolidating the staff records.
